Thomas Hoffman, MD FAAFP

Physician

Dr. Hoffman graduated from Wayne State University School of Medicine in 1982. He completed his Family Practice Residency at Saint John Hospital, in Detroit Michigan, in 1989.

​

Dr. Hoffman has been in private practice in Hastings since the summer of 1989, when he and his family relocated to the area.  He also works with the Barry County Community Mental Health Agency substance use disorder program, and runs a medication assisted treatment program for persons with opioid use disorders. He is part of the affiliated clinical faculty for the Kirkhof College of Nursing at Grand Valley State University.

Della Hughes Carter, DNP, RN, GNP-BC

Nurse Practioner

Dr. Hughes graduated with a Bachelor (1993) and Master’s degree (2000) in the Science of Nursing from Michigan State University and her Doctorate in Nursing from the University of Southern Indiana (2015).  She is Board Certified and a member of the American Association of Nurse Practitioners.  She is an Assistant Professor at Grand Valley State University with multiple national publications and presentations.  She is the only Nurse Practitioner to sit on the State of Michigan National Kidney Disease (NKF) Advisory Board.

Della was an Assistant Professor at Michigan State University since 2004 and worked as a Nurse Practitioner at Carefree Medical in Lansing, MI.  As a nurse practitioner she seeks to provide a partnership with the patient in the management of their healthcare, with a focus on inter-professional practice.

Barbara Ketchum, RN, CM

Care Manager Nurse

Barbara was born and raised in Lake Odessa until age 14, when she moved to Cadillac MI. She received her Diploma of Nursing Degree from Butterworth School of Nursing, in Grand Rapids MI.

​

Her nursing career has provided opportunities to work in several areas, including med-surg, family practice, surgical nursing, outpatient services, oncology, home care, and case management.

​

As a case manager, she helps to educate patients about their illnesses, and assist them in being proactive in making healthy lifestyle choices.


Outside the office, she enjoys spending time with her family, reading, gardening, camping, attending local theater, and being involved in her church.
